We have an awesome thing going here in Massachusetts (NARH)...every
breastfeeding mom sees either the IBCLC or myself (CLC...who works under the
IBCLC...in constant contact) everyday at the hospital (48 hours for vag.
delivery and 96 hours for c-section).  

Once dishcharged, they fall under "Berkshire Nursing Families"...we provide
home visits, breastfeeding support line, phone support, support groups,
sling lending, bf classes, email support, hospital clinic, pump rentals, etc
  We contact moms until 6 months old (or they discontinue bf) at various
times depending on the age of baby and any issues that come up. (ex. mom
just going home gets a call the next day, whereas a mom with a 5 month old
doing well will receive a call 1 x/month.  We send out emails with with
various info...growth spurt reminders, baby development/behaviors, etc.
specific to certain ages (ex. 3 months, 4 months, etc.) or mailings for
those w/ no email access.  All but the pump rentals are free to the client
if they deliver at NARH or live in the service area (Northern Berkshire
County).  



Most moms get a visit from the Visiting Nurses Association within a couple
days of discharge as well.



We are in the process of developing a breastfeeding dishcharge bag.  Many
companies have donated wonderful stuff (magazines, books, tea, ice packs,
lanolin, etc.).  We just need to get creative on how to pay for the bags
themselves (the most expensive part).
