From the weight gain charted above, I do see that *something* happened when
baby was between four and five months old.  When I hear that baby is only
eating five times a day at that point, it seems to me that mother's body was
not experiencing "low milk supply" exactly but was supplying milk for a baby
who only eats five times a day - not usual for a four/five month old, whether
breastfed or not.  If baby only eats five times a day, essentially the
mother's body thinks the baby is weaning.  It may take a while to convince the
mother's body to gear production back up, if indeed her body can be convinced.
    Was this baby getting any solid foods?  Was mom giving solids first before
nursing or instead of nursing?
   Was baby sick with any GI illness during month four or five?  I have seen
babies lose quite a bit of weight that way.  My son lost a pound in a week
(weight was 22 lbs or so at both 12 and 14 months) because he was so ill, even
while nursing constantly throughout.  It wasn't that he hadn't gained any
weight but that he had lost and gained about the same in the same time period.
  Other thoughts:   Was mother trying to hyper-schedule the baby?  Sounds like
an Ezzo plan:  feed at 9am-12noon-3pm-6pm- and 9pm then put the baby to bed.
Is there a chance that the mother felt as if a four month old should no longer
be demand fed and so baby was fed according to the clock?
  Was baby already receiving bottles of anything before four months?  This
just occurred to me, that it would be unusual for a baby who was exclusively
breastfeeding for four/five months to be willing/happy to take a bottle of
formula without a fight, unless he was *very* hungry due to scheduled feeds or
had been taking bottles (or pacifiers) extensively?

Just some random thoughts.  I hope it all works out for the best.
