I am a private practice LC in South Florida, USA. Recently, I attended a
physicians' breastfeeding task force meeting for one of the large hospital
systems in our area. As might be expected, the focus was on the many
roadblocks to breastfeeding encountered in the hospital. During the
discussion, an OB suggested that having parents sign a consent that noted
some of the risks of ABM feeding might serve as a way to 1) give uninformed
parents pause before giving formula and 2) track which nurse was responsible
for giving the parents the formula. This OB thought that having the nurse
witness this kind of consent would also give the nurse reason to pause
before offering ABM, as several pediatricians in attendance felt that
particular nurses seemed to offer ABM to moms more than others.

From this discussion came a question, which I am also posing here:

Is anyone else (meaning other hospitals) doing this? 

If anyone has any experience with this kind of consent form, please share it
with me, either off-list or on. 

I can't tell you the fascinating things I learned in that hourlong meeting!
