If her nipple is remaining misshapen for some time after pumping it sounds
like her nipple is being damaged. Her pumping technique may be incorrect
[nipple off center in the flange], flange tunnel is too small or too large,
too much suction and/or pumping for too long. I've seen this with pumps that
don't self cycle form the mom maintaining the negative pressure on her nipple
for long periods. If she has a Medela mini-electric it self cycle. The manual
releases if she tries to pull too hard against the breast.
The nipple refusal could be due to a few factors: imprinting--the baby just
doesn't like a different nipple, the *swelling* may make it so the milk does
not flow and/or the nipple and areola is too hard to compress.
