This mom needs some empathy and emotional support first, as she is 
probably horrified and terrified that her child developed rickets, and 
is in the anger stage of the grieving process.
Once she feels understood and heard, and knows that we are taking this 
issue very seriously, then she might be open to education that formula 
is not necessary, only extra vitamin D is for babies who cannot get 
enough sunlight in a safe way to meet their needs. You might also want 
to praise her baby's pediatrician for picking up the rickets very early, 
when it is easy to reverse.

I am still an active LLLL. At meetings, I educate moms about vitamin D 
and encourage them to assess their own infant's individual risk when 
deciding whether their infant likely needs vitamin D supplementation. In 
NYC's lattitude, we can only absorb enough sunlight for vitamin D 
sufficiency from May through October, so this is definately a concern. 
Moms who have darker skins or who have darker skinned infants, moms who 
live in neighborhoods where it is not as safe to go outdoors, moms whose 
religious traditions prescribe full body covering, and moms with chronic 
illnesses who stay inside more all have increased vulnerability to low 
vitamin D stores themselves and rickets in their infants.
