Dear Pat, I follow the manufacturer's directions:
< 2 weeks of age: 1/3 dose/day or full dose q. 72 hours. ( I think it is
easier for mom to remember 1/3 dose daily)
2-4 weeks of age: 1/2 dose daily or q. 48 hours (ditto)
The med comes as 10 mg/ml.  The loading dose is calculated at 6
mg/kg(baby's weight) and the maintenence dose is 3 to 5 mg/kg/day for 14
days.  For less than 4 weeks I use the 3mg/kg dose  (divided 1/3 or 1/2
whichever is appropriate.)  In other words you figure dose on baby's weight
and then divide it in 1/3 or 1/2.  Is that clear?
I usually give mom a 3 cc. syringe (with needle removed) and stick a piece
of tape on for a marker so I'm sure they give exact dose (since one dad
used a teaspoon instead of an ml !)  call me if I can be of any other
