Lactose-free formula is NOT hypoallergenic.  It is cow-milk based formula
with the lactose removed.  Soy-based formula is just as allergenic as cow
milk.  The hypoallergenic formulas are Nutramigen, Alimentum, and
Neocate--the proteins in these products are broken down so they are not
allergenic.

The physician who recommended lactose-free formula to this infant/mother
apparently doesn't understand what hypoallergenic is.

Working with the mother's milk supply and diet and the baby's nursing
effectiveness and intake would be an appropriate way of managing the case.
