hi mirine,
you said "making it hard for me no to just advise her to give up and bottle
feed." i know what you mean, altho i'm sure you did not mean this literally.
in a recent post, i said that i think we (BF advocates of all people) should
not be the one to recommend weaning (unless a medical contraindication such
as galactosemia is revealed), but rather the mother is the one to have to
make this decision. for example, i am working with a dedicated 17 y.o. mom
who has a 26 wker at another hospital. she asked me when should she stop
expressing, as it is "getting old". the baby is not quite 3 lb yet. well,
you can imagine, i gave her a pep talk, encouraging her to continue and do
kangaroo care and ask if the docs will let her put baby to breast to begin
that transition. i'm not going to be the one to recommend weaning.

there are some times when i feel i have "had it" with a particular client.
this may sound crass, but not wanting to hear, yet again, her reasons for
not following through with the plan you laid out together, it is tempting to
say "why don't you just bottle feed then." that is just human nature and
frustration setting in. but we cannot allow ourselves to be that crass, we
are professional and put our "feelings" aside, while trying to "act" in the
right way. and we must realize that what we recommend so easily (pump,
alternate feed, try this shield, use this sns, etc) is MUCH EASIER SAID THAN
DONE.

also you said "The twins won't latch and have developed a nipple preference
for the bottle." i think this is due to the low supply, which recent posts
have addressed, saying if the supply is abundant, the baby will drink.
