Oh, Jan, what a sadness. I had a case that was similar a few years ago.

Mom had fed her first baby without problems. With the case I was called in
for she had similar output as Jill has. After the measures that I
suggested failed, I suggested an ultrasound for retained placenta. OB was
highly offended and said that she examines all placentas thoroughly. There
was no possibility of retained placenta. And, as with Jill, the only
symptom was lack of milk.

Mother struggled for a few weeks, feeding formula, and gave up trying to
get a milk supply going. Then at about 4 weeks she had a large bleed and
an ultrasound revealed a tiny placental fragment. The next day there was
the beginning of a milk supply. The mother went on to get her supply to
adequacy, but this was after 4 weeks of needless formula. If she hadn't fed
her first baby I doubt that she would have worked to build her supply.
