The only thing I would say differently than Laurie is that I find beginning with 6 hour blocks (with maybe a small bit of expressing on unused side during 6 hours) seems to get better results. I find it takes about 2-3 days to tamp supply down this way. 
Remember the days when undersupply was our biggest problem? 
I think reflux in BF baby is usually oversupply. Also extremely fast weight gain early on may be a result of oversupply. Is extremely fast weight gain in a BF baby same as formula fed baby and a harbinger of obesity??? Wish I knew how to research that one! Pat in SNJ
