It is usually necessary to treat mom and baby for yeast overgrowth because one of them is having symptoms. Many docs do not treat thrush in babies because it tends to "come right back".  If mom is having symptoms (both nipples and areolae where the baby's mouth touches are red and sore) then both should be treated.  I believe there are regional differences in Nystatin resistance.  I use 1/2% Gentian Violet, and have not seen it fail in 13 years.  In my opinion, if it did not help, it is not yeast.   Vasospasm would be my first guess.

No, I would not expect APNO to treat the baby.
