Susan,
Try breastfeeding in prone with the baby laying on mom's chest, lower
than the breast so he has to "climb" the breast with his lower jaw.
This will give a nice asymmetrical latch, and will help the tongue stay
out of the airway, and  will provide head extension to help open the
airway.  If you've ever seen the video "Delivery Self Attachment"
that's the position I'm talking about.  Very short, very frequent feeds
are helpful as well for infants with difficulty maintaining the airway.
