Have you tried remedial co-bathing.  My daughter had similar problems.  
During the first bath she sucked correctly then went back to the incorrect.  Over 
the next 3 weeks we bathed together frequently and gradually, but surely, she 
would suck correctly for longer periods after the baths.  After the 10th bath 
(at 5 weeks of age) her suck remained correct and she is now 8 months. I was 
also doing oral exercises and putting pressure on her chin during feedings but 
they had little impact.   In addition, I never bathed with my son, who had the 
same sucking problem but, never learned to suck correctly.  With him the pain 
went away at around 6 weeks.  (I didn't realize what was wrong with him as I 
wasn't an IBCLC at the time).  

FYI, for those interested in co-bathing.  I continue to co-bath for fun with 
my daughter and I noticed that at about 3-4 months the reflexive 
rooting/sucking behaviors ceased. Now, at 8 months, she still enjoys latching on and 
nursing in the tub though. She never did the crying patterning that Nicki describes 
but we were not seperated nor was there any breast refusal and she was 
thriving and transferring milk even though sucking incorrectly.  I find the impact of 
co-bathing fascinating and it was a healing experience for me since I had to 
have a c-section.  Chris
