Rob,
Would this mom be willing to try bf very frequently for very short
periods of time?  Often with babies with cardiac problems, the problem
is cardiorespiratory endurance.  Short, very freq feeds (several in an
hour) can sometimes overcome that problem, and keep the milk fat high
enough so that baby gets more calories as well.  This approach has
worked for some babies with congenital heart disease in my practice.
