Sound like what we used to call (or still call) "ductal spasms." Many of us
have found that some mothers have some pain, sometimes attributed to spasms
in the ducts, *after* each nursing, and may be related to "refilling of
ducts" (BAB, 3rd Ed, page 511.) This seems to be more common in mothers
with OMER. The baby is only two months old, it is doubtful that her OER is
totally rectified, as this is something most mothers who have it struggle
with for quite a while. Some reports seem to say that these spasms are more
painful with the first baby.

There isn't a lot of published data on this, mostly anecdotes.

Rest fluids, frequent nursing, some warm compresses and perhaps a mild
analgesic, like Motrin, may help with the pain. Some mothers also find it
painful enough to use labor relaxation tecniques breathing.

As you did mention, make sure positioning is not straining her,(using
pillows, make sure she isn't "hunching" ect) and that the baby is taking
enought tissue. Her reports and observations alone, if she has never nursed
a baby before, may not be enough, as she may not know what is "enough
tissue" or what is a proper position. I have seen too many mothers not even
realizing they are in horribly uncomfortable positions, until someone sees
them and points out not only what she is doing wrong, but what she could be
doing right.  She may need either a home visit or an IBCLC consult, if this
continues. There is a lot that can be done, lactationally speaking,  before
sending her off for expensive treatments like acupuncture or chiro!

(Make sure you assess for plugged duct and mastitis, also!)
