An ankle sprain that is not what it seems:  Tendon rupture, such as the
Achilles tendon, can occur from a relatively minor trauma. It is more
common in the elderly and those taking corticosteroid medication or
"being treated for an infection with a fluoroquinone antibiotic, such as
Cipro."

It went on to say "if you experience sudden *pop* behind your ankle
(or anywhere else in your body), call your doctor right away and make
sure he understands what you felt."  Tendons must be repaired by
an orthopedic surgeon no later than the next day, as it becomes more
difficult to pull the ruptured ends of the tendon together again to
join them.
