I'm working with the mother of an 11 week old. She is a CLC and has successfully breastfed one older sibling. Latch is painful, shallow (variety of positions and latching techniques have been tried), lips tucked in (when they are pulled out, he flips them back in again). Mother has very forceful letdown (which could explain the shallow, clamping latch), there were symptoms that pointed to oversupply, but besides discomfort/gassiness, this seems to have been resolved.

Mother observes that his smile is crooked (right side of baby's mouth turns more down while left turns more up), and suspects dome palate - her MD (also a CLC) confirmed this, but the IBCLC doesn't think it is. Baby's chin is recessed. He has been diagnosed with reflux and prescribed zantac. When on the zantac, the consistency of the spitup did change, however the frequency didn't, and his comfort level remained the same. Mother suspects gas as source of discomfort.

Baby is gaining VERY well - he's quite the chunky monkey - but mother is experiencing cracked nipples, peeling, blisters on tip, and bruising where nipple meets areola.  Her nipples are somewhat flat - the right more than the left, but latch seems to be the same on both sides.

This poor mother is so dedicated to breastfeeding her son, but she is in so much pain and experiencing so much frustration... Any help, insight or suggestions you can offer would be GREATLY appreciated!
