I confess to not having taken the time to research on my own, but you hit a real nerve.  Probably one of the biggest frustrations in our work (especially us outpatient LCs) is the devoted, committed mom who has primary insufficiency with (or without) the signs of insufficient gladular tissue, etc..  Are you aware of any studies exploring treating women who show signs of probable trouble prenatally with dom, etc.?
These moms' sadness, frustration, and guilt really tear at my heart.  Even if we could give them hope that for their next baby there might be help available, it would help them (and me!)
