Two positions that may work for this little guy are having him prone on 
mom's chest so his head is way extended, and he has to "climb the 
breast" to latch.  This will bring his jaw and and help a tiny bit with 
the tongue as well.  The other is sidelying with him lower than mom's 
breast, so he has to tip his head back a  lot to latch.

Mom is doing just the right thing by expressing and keeping her milk 
supply going, feeding him and trying to help him latch.  If you have 
access to an electronic scale that is accurate to a few grams, you can 
do pre/post weights with the nipple shield to ensure that he is 
tranferring milk through it.  If he is getting milk, you can just let 
him nurse with the shield until he grows.
