A colleague has asked for help on this situation, and I've read the archives on galactorrhea/hyperprolactinemia:

Low-income mother 7 months pregnant with her third child.  The second child is 11 years old.  

The mother has complained throughout her pregnancy about being uncomfortable with milk in her breasts, and was expressing.  She also apparently "borrowed" a neighbour's baby to relieve the engorgement (yes, well...) and suffered alarming uterine contractions.

She is wearing cloths inside her bra to soak up the leakage - if the breast is merely touched, it squirts milk.  I have no information yet on what her other lactations were like.

The treatment options seem limited because of her income.  I don't know if she will have access to any sort of hormonal testing (pituitary, thyroid).  Where shall we go first?  What's most vital to do?  All we've suggested at this stage is to try cabbage leaves for the discomfort.
