Pam:  I'm not going to revisit all the yeast discussion, but it is highly
likely that this is very deep in the duct and that is why pain doesn't arise
until end of feeding.  Have you tried (there are many more listed in the
most recent issue of MOTHERING Mag):  warm compresses  unless inflamation is
obvoius; very deep breast massage with olive oil (antifungal properties as
well as lubricating for deepest massage) and use of deep breast compression
during actual nursing; lecithin (actually is a fat emulsifier and really
assists in flushing anything sticking in ducts); ibuprophen to reduce
internal inflamation (again assists in draining); vitamin C with lots of
fluids (again, helps flush);  grapefruit seed extract; maitake tea; longer
course of Diflucan (loading dose of 400 mg, with 200 mg daily for 21 days)
or oral Nizoral (must be monitored and cautioned regarding liver toxicity).
After or concurrently with all the above, a multi-strain Lactobaccili
capsule (build up to 2 caps 3 TID) with FOS should be used for several
months.    There are many holistic means of treating yeast, but if the mom's
practitioner will work with her, I would use a combination approach for the
fastest response.  'Hope this helps.
