Re the recent discussion of mentoring, I actually have a book on mentoring
on my desk right now. In fact, I'm currently working with a mentor (in the
field of instructional design, not lactation). Sometimes, as part of what
we do, we help clients establish mentoring systems. Thought you might be
interested in a reference book that my mentor uses when setting up these
programs. It's called:

Beyond the Myths and Magic of Mentoring: How to Facilitate an Effective
Mentoring Program

The author is Margo Murray with Marna A. Owen.
Copyright is 1991, ISBN# 1-55542-333-7

Though it's geared to setting up mentoring systems in business
organizations, it also has information for anyone who's thinking of
entering the mentor/protege relationship. Some of the chapter titles are:

Payoffs and Penalties for the Protege
The Mentor's Motivation and Concerns
Structuring the Mentor Role
Selecting Proteges and Diagnosing Their Developmental Needs
Negotiating Sound Mentoring Agreements

The chapter on negotiating mentoring agreements has a sample "Mentoring
Agreement Form" which you may find useful.
