I may be going out on a limb here, but it seems to me that food 
sensitivities could be a factor.  Many times the main symptom is a 
feeling of thickness, itching, and/or swelling in the mouth and 
throat, and this can definitely cause gagging and choking behavior.

I know a number of people who are old enough to clearly describe 
things, who report this kind of reaction to foods they are sensitive 
to.  The appearance of the choking problem at 6 weeks also fits in 
with a food sensitivity, as there is usually a problem-free period of 
a few weeks before food sensitivities manifest themselves.

The mother could try an elimination diet and see if it helps. 
Exactly what she eliminates would be dependent on family history, or, 
if there is none, she could start with the most common trigger foods 
-- dairy, soy, nuts, shellfish, etc.
