Have also used 24 hr only hydrogels with apno etc.  Put a dab in the middle and spread, then apply to the nipples.  Wipe off pad and reapply after each feeding, then discard after 24 hours.

Yes, I have used it with APNO or mupirocin but always cautioned mothers not to wear it at night since they might not remember to remove the saran wrap before nursing in their exhausted state!  Lately, I have just suggested breast shells.  They keep the nipples from touching anything so the ointment doesn't rub off.
