Abx after feeding, sparingly so it is all gone by next feed. I tell mom she is not buttering bread.  Air dry prior to abx.3x day.  Personally  I love normal saline soaks 3 x day.  I think they work very well.  I cup water and 1/4 tsp salt.  If it stings, she was too generous with salt!  Now telling mom to use abx and /or saline soaks are really prescibing......where do simple things like this and using OTC meds , normal saline fit into the grand scheme of practice for IBCLC.  As LLLL I can say many mothers have found......as NP I can say use this or that.  Sticky situation, when we know what will probably work and her dr or ped doesn't have a clue.  Another thing we can do is tell her to call her dr and ask if she can use xyz. What are other opinions?  Liz, where does this fit???  Thanks, Pat in SNJ
