I hesitate to add this, as a fever in a baby refusing to nurse might
definitely be due to dehydration and this, of course, should be ruled out
before considering other, less dangerous situations.  I know that 3 mo
might be a little young, but my 5mo has just gone through a week of off and
on breast refusal and low grade fevers. It seems to be teething related as
she is otherwise happy, chewing on everything, urinating and stooling and
drooling like mad.

Might another possibility be an illness that includes a sore throat?

As I stated previously, rule out dehydration first.
