I don't know if this will help, but..... I had a mother who also had a
recurrent white dot on her nipple. She also often got red and streaky on
that quadrant, and had mastitis a couple of times. We did a combination
of things, including having her nurse the baby on the floor with her on
"all fours" once a day, and also taking care to nurse the baby a few
times with the chin pointing to that quadrant for better drainage. This
reduced the frequency but didn't eliminate the problem. Her white duct
was more like a blister, and repeated problems had created a small
crater. After some research, I had her add a neosporin ointment treatment
in a sparing amount after feeds, and finally she cleared up after one
more flare-up. Evidently a secondary infection had been preventing
healing despite otherwise good management.
Not all of this exactly parallels your mom, but perhaps something here
might be useful to her.
