Oh- now that makes perfect sense.

Yes, and she was using cross cradle hold! The swelling was at 12 o'clock and at 6 o'clock. And it felt just like water (under skin)  

Thanks so much for sharing your plan for the other mom. We were already doing some of this but I was unsure how soon to go to every 3 hours or more. Lying on her back and using massage would be a great idea. We also located an osteopath in the area that does lymphatic massage for breasts. But her engorgement is diminishing and her breasts are not quite as full, so she hasn't done this yet. She is more comfortable than with her previous children, too, so she is pretty happy about that. 
You know, it does seem to me as though she just has a whole lot of milk producing tissue. Someone else suggested gigantomastia, but my understanding of that (which is limited!) is that the connective tissue is what enlarges so much, and I'm not sure that's the case here.
