Here is what I would look for: poor positioning and latch which can occur at
this stage, like baby pulling at base of nipple, where it meets the areola,
forming a crack. Also a nipple bleb which is extremely painful, but often is
associated with a blocked area behind it. Also vasospasm because warmth
helped.
