Donna, I had a patient(about 10 years ago when I first worked as a hosp.LC)
She had been burned on the chest as a young child.  The left nipple was
totally occluded with scar tissue.  She got very engorged on the left, but
continued to nurse the infant entirely on right for 8 mo.  I had the
opportunity to see her with baby #2, and #3 at the same hospital.
Interestingly the engorgement on the non-working breast was less with each
new baby.  I was aware of the Chinese research on women who "culturally"
nurse exclusively on one breast and experience increased Ca, so we talked
about her future need to pay special attention to BSE and mammograms.

will forward my extensive Bib on early initiation of BF.

Re: posting about program for young moms - I'd just politely check with the
program.  I imagine they promote BF.  If not,maybe there is something you
can do to help them add this.  Remember you were reading the mom's opinion
on what they taught.  If bottlefeeding is her cultural norm this is what she
focused on.  Frankly, I wish a lot of cities were so proactive about meeting
the needs of these young moms.  Remember their families and experiences
aren't like my family or yours.  I work in the inner city and see where
heaps of teaching would help,but I have time and scope of practice limits.
An agency that I am Bd. member has a very sucessful program of pairing
volunteer moms with new, young, disadvantaged moms.  Their goal is to help
her prevent the second pregnancy (statistically most young moms have another
baby within two years) and teach her parenting skills.  If you haven't been
adequately parented you don't know how to parent!  The latest figures after
one year, are that 196 of our moms are not pregnant, 4 are.  This is
impressive!  I currently have a 16 y.o. mom with an 8 month old.  Her 30
week 3 pounder is in NICU.  Two babies in less than 8 months!
She has tried to pump for the new baby, but needs lots of help.  I referred
her to our WIC BF counselor who is great.

I've been following the thread about BF promotion in the public forum.  I
really like the one who asks moms and gives her card.  I do something
similar at church. Honey definetely works better than vinegar.
