Hello, I have yet another medication question. We have a mom in our hospital who just delivered her first child. This mom suffers with severe chronic back pain, and was on demerol during her entire pregnancy in very high doses. We are currently watching the baby for withdrawal - so far, so good. 

This mom will be going home on Lortab at the dose of 20 mg every 6 hours. The usual dose is 5 mg every 4 hours. Dr. Hales book indicates that the latter dosage should be no problem. I am worried about getting way too much drug through the milk due to the dosage of 20 mg being 4x higher than the regular dose.

Just wondering if anyone else on the list has dealt with mothers who take large amts of these types of drugs for pain, and how the babies do. Or if I should be encouraging her to pump and dump until her dosage begins to decrease (at some unknown time in the future). THANK YOU!
