If baby has gained weight well, have her feed on just one breast for
several feedings in succession. I have a colleague, who is also a former
client, who had a similar problem, compounded by an extremely active
let-down reflex. She fed her baby for 12 hours at a time on one breast
before switching. She did this for 2-3 days, and then went back to using
one breast for 1-2 feeds before going to the other breast. This had the
double benefit of reducing her oversupply, AND allowing her baby, who was
beginning to refuse the breast because of nearly drowning in milk at every
feed for three months, to trust and enjoy the breast again.
To avoid the risk of plugged ducts and/or mastitis, your client might need
to pump/express SMALL amounts from the unused breast just for relief in
between switching.

Subject: painful oversupply

I have a mom with a 4mo that is continuing to have painful oversupply.
She continually gets engorged.
She has an auxillary breast under her armpit that gets full as well. She
is back to work 2 days / week and pumps 2-3 times while at work. When
home baby nurses about 6 times/day and she leaks enough while baby nurses
to fill a 3 oz bottle. She wants to quit because she is tired of always
being in pain. There is no sign of thrush or anything else I can think of
that might be causing this. Any words of encouragement I can give her or
solutions to her problem? Thanks for input.
