I think the mother was using a pump NOT MEANT FOR THIS SITUATION and her 
supply down-regulated over the first couple weeks.

I would advise the mother to get a top-notch rental pump, I favor the Medela 
Classic. I would observe her pumping and make sure she has the right 
breastshield fit. I would suggest to her to get on an herbal remedy, perhaps 
consulting an herbalist or using the More Milk tinctures (motherlove 
herbals). I would advise her to pump at least 8x each 24 hrs. I would advise 
her to have her health checked out thoroughly like diabetes, thyroid. Of 
course, counsel her on all of this gently, taking into account her life 
situation and stresses.

How is the baby now? Was the wt gain a result of sepsis? some other illness? 
or was the baby just consuming too little volume (almost always the case).
