My first daughter ( who would have been 30) was a 32 weeks premi that I
wasn't allowed to even touch for the first month of life . No one ever asked
if I was planning on breastfeeding, the nurses just brought me to a room
with this huge old machine and told me to pump for my baby, then left. No
instructions.
The flanges were a bunch different then now and  being fairly small breasted
I hooked the pump to my breast and turned the pressure all the way up. No
joke- the machine "sucked" my nipple into the smallest part! My nipple now
was about 6 inches long and squeezed to the diameter of a pencil- trapping
me. After much screaming, the nurses showed up and had to finally use a
lubricated surgical q tip and mineral oil to push me free!
Needless to say, when I finally did nurse my baby, no one "taught" us. We
just worked it out together. ( At least I never had a problem with inverted
nipples :)
Side lying? pure exhaustion was my instructor. I often work up  with baby
hicky marks along my sides as they searched for their midnight snack.
