Fleur's mother writes, "I did not do the full protocol (i.e., with birth
control pill), so I never achieved a full milk supply."
[NB] 
While you are talking with this mother, Fleur, please please please dispel
the myth that adequate milk supply can only be induced or re-established if
the mother uses a hormonal protocol.  
While working with adoptive mothers overseas, I have seen them move from no
milk (last breastfeed 6-8 years prior) to a supply sufficient to exclusively
breastfeed a newborn (6-8 weeks old) in under a month with no hormonal
protocol and no galactagogue - also no pump and no supplementer.  The
protocol is simple - carry the baby for most of the day; sleep next to the
baby; put the baby to the breast as often as it will happily attach and
suckle.  Use a cup to supplement small amounts 8 - 10 times a day, always
after a breastfeed.
