A mother posted to me on a pediatric website I work for. The title of her
post was "Breastfeeding with Low Prolactin Levels". I think it would be
easier to just post her initial post and her follow-up (her answers to the
questions I posed to her in order to get more information). I have her
permission to do this. I have checked Riordan and Auerbach and although there
is some information there on Prolactin levels, I really couldn't come up with
anymore to tell her. I also searched the archives with the same results.

I have already given her the information from Dr. Jack's handout on
Domperidone and told her of its availability from Mexico or Canada (will need
to get specifics on that). I will also suggest the SNS at breast. Even if
there's not much milk available, my thoughts are that she might benefit from
and enjoy the baby just being at breast

Please let me know your thoughts on this. Any and all are very much
appreciated!

Following is her initial post and her second post in that order:

This is such an emotional thing for me...On 2/13/00,I gave birth
to my third child. My milk never came in for my first 2 babies, however, with
my first I tried to bf for 2 mos. An endocrinologist tested my prolactin
level while I was pregnant with my second child and as it turned out my level
was extremely low.He knew of only 1 other such case in med. literature.I
did not try to bf baby#2 as I became very depressed with the
failure to bf baby#1. Six years later I had baby#3. My prolactin level was
o.k. in sept99(4mos). When I went in to labor(2/12/00) the O.B. suggested
that I try to bf as my sept. level was o.k.After my son was born I requested
a prolactin level test(all the while I was breastfeeding) The level was 14
and I guess is supposed to be 29. I was very sad but decided to
try a regimen of reglan, fenugreek and blessed thistle. I did get a little
milk but by this time I was supplementing with formula (avent bottles) My son
became frustrated and would not nurse . I think whatever milk I had was not
much at all and not increasing. I gave up but continue to wonder if
things might have improved. Another drug maybe? What is
domperidone?Any thoughts/advice are really appreciated. I would even try to
relactate. My son had blood in his stool and my pediatrician said that it was
probably a milk allergy so he is now on a soy formula.I would really like to
be able to breastfeed. Thank-you for any advice.

Thank-you for your help! Answers to quests: no trouble conceiving (between
#1and#2 I had 2 miscarriages,that's why I saw the R.E.),no
hormonal therapy,no other hormanal probs that I know of,no placental
retention that I know of,no abnormal bleeding after birth, slight
enlargement of breasts during preg(I am very small breasted), possibly dried
colostrum on nipples during preg?,some breast fullness, tingling days
after birth.I did use a manual pump a few times and did see a few drops of
bm.thyroid stim test 2x,first while preg w/#2,second after his birth
(R.E.),results normal if I remember correctly,it was 6+yrs.ago. Yes,posting
on Lacnet o.k. w/me. L.C. andC.N.M.@ hosp. were very helpful
and supportive and recommended reglan and herbs. I wonder if either one
already posted s.t. on Lacnet? I would like info on obtaining
domperidone. Do you think it has a higher success rate than reglan? Thanks
again.
