having the same crease and showed me, he said one of their other children
had this also and another has identical ears to the baby and none have had
any health problems except a brother who had hypospadias which was
corrected with surgery. I was amazed that the doctors were not concerned
about this baby, I recommended follow-up and suspect he needs to be

I have a feeling the midwife's first impression might not be too far off.  
Perhaps a thorough genetic screening ASAP might be helpful here.  Mosaicism, 
which is rare, occurs when nondisjunction of the 21st chromosome happens in 
one of the early cell divisions after conception.  The individual born would 
then have in his body cells with both 46 and  47 chromosomes.  If this is the 
case, other family members may have to be tested as well.  Follow your gut 
feelings.
