Forgot to mention this in prev.post.  Bound in Kittie's product book is a
form to report "medical product" which have harmed a patient or caused
adverse reactions.  pumps would be included.  There is a form and detailed
instructions on submitting the form.  If you would like to request a form or
make a report, contact the FDA at 1-800-FDA-1088, fax 1-800-FDA-0178.
