There's no good way to "measure" the nipple.  We eyeball it.  Most common size used is #2 or 3.  When you use the cup, it will gather up a little of the areolar tissue.  Refer to the Journal of Clinical Lactation for an excellent study on the use of these.

Subject: Inverted nipples, supple cups



I want to thank those of you who responded to my inquiry re inverted nipples. The mom I am working with has been faithful to pumping and nursing and used suction cups to help evert her nipples.mher supply has increased greatly on follow up visit. She transferred 5 oz by wt nursing through nipple shield and will try to decrease supplement. Her baby gained 8 oz in 5 days and is greatly reassured now. The manufacturer of the supple cups does not specify how to measure nipple size, by inside or outside diameter of nipple?  They come in 5 sizes #1-5, diameteters 11, q12.5, 14.5, 16.5, anyone know?  Interestingly, her mom I believe has a tt baby but I was not there in consult with her really, asking me questions though.
