While we still have to fight the battle over lengthy NPO before
surgeries that require anesthesia, most of our docs aren't making
any restrictions when it comes to circs.  We used to have a lot that
would say "don't feed the baby (regardless of breast or bottle) a
half hour before".  Now the only one that says that is the one that
likes to dash in, do it, and dash out.  He wants the boys all lined
up and ready to go rather than it having anything to do with concern
about baby having an "empty" stomach.  Some that used to worry about
the baby vomiting during crying have found that this rarely occurs,
even if baby was feeding immediately before.  A bulb syringe is
always right there and the nurse or assistant helping hold the baby
is readily available to turn baby's head or whatever is needed in
those rare instances.  Many are becoming more sensitive to baby's
feeding.  They tend to make rounds with the moms first (here, the
OBs do the circs), let her know the circ will be done as soon as
rounds are over.  If the baby is at breast when the time comes, the
docs will usually finish other duties first.  When the circ is the
last thing the doc has to do before leaving and baby is nursing,
some will give the parents the option of waiting until the next day
unless they are going home that day.
So, in summary, babies are not required to be NPO before a circ and
most docs try to be sensitive to the baby's immediate feeding
status.
