Per the ABM's protocol on NICU Graduates Going Home,

"(Note: Many neonatologists and institutions add powdered discharge
premature
formula to expressed breastmilk to provide enriched feeds while still
providing the
advantages of breastmilk. There is no evidence to recommend for or
against this practice.
This use of powdered premature formula is off-label and the potential
for error is great,
so be advised to be extremely cautious if using this approach.) "

And from Hale & Hartmann's Textbook of Human Lactation, page 238:

"If supplementation is deemed necessary, breastfeeding can be spported
by having the mother directly breastfeed, then substitute from one to
four feedings (as calculated for nutritional needs) per 24 hours of
preterm or post-discharge enhanced formula to reach growth and
biochemical goals (Academy of Breastfeeding Medicine, 2004b).
Alternately, powdered follow-up formula can be added to expressed
breastmilk as a fortifier."

There is no reference given in the Hale/Hartmann text for that
particular practice, nor do they list the warning that the ABM does in
their protocol.

Hope that helps this conversation along (I have not read every post in
this thread so perhaps this has already been brought up by others)....
