I have an interesting patient and I was wondering if any of you out
there have had a similar patient or advice. She is a 19 year old
pregnant patient who has had an "abscess" on her right nipple for 4
years. Limited access to health care. The "abscess" appears in the
region of Montgomeroy's tubercle and is about 2cm in diameter and depth.
Very tender. I lanced the area as near the periphery as possible and
placed her on Dicloxicillin. I also left a "wick" in place as she has
self lanced this many times in the past only to have it recur. It looked
much better 48hours later. She has since gone on to deliver and is
initially doing well. Baby is nursing on the other breast. I felt
that though an incision near the areola is not ideal for breastfeeding
neither is a large abscess. Anyone with a similar experience out there?
It was not a "typical" breast abscess in the paranchyma.
TIA Pamela Berens OB/GYN
