Re:  Kathleen Bruce's ineffective nursing baby.  Just a few ideas.  How long are
mom's nipples?  Are they so long they perch baby out so far that jaw closure
must compress nipples?  If so, time and good growth may assist.  No matter what
the cause, the mom would do better (using her precious time) to make an
executive decision after about 10-15 min. of this ineffective nursing to just
stop and pump.  Without so many long nursing sessions (which appear to be
ineffective both in terms of intake and stimulation) she could pump more.  Also,
try supplementing first to see if a jolt of energy would wake up the baby and
put more power into his sucking.  Also, consider pedi check to rule out
underlying heart defect.  Another possibility is reflux which makes baby
self-limit intake.  Baby perches out on tips, compressing nipples deliberately
to stop flow.
