My hospital uses a consent form for the pacifier.  It is taped to the infant's crib and is updated with initials and date if mother changes her mind.  I can get the exact wording when I'm at work and post it later.  The form gives the hospital permission to use a pacifier anytime to calm the baby, or to use a pacifier with a sucrose solution during painful procedures only (like a circumcision or blood draw), or no use of a pacifier at all.

The problem has been that a parent will give permission for a paci to be used during a painful procedure, and the paci finds it's way into to the baby's mouth for a HepB shot, then stays in the crib to be used again during assessments, or essentially, ad lib.

Now that we have started using a sucrose solution for painful procedures, I'm seeing it in the mother's room and hear that a nurse used it to encourage baby to latch.  (Hardly ever is it successful.)  It's always something!
