Hi!  Having been a LDRP RN for many years and having worked in a hospital
where we had excellent neonatalogists who allowed babies to stay on Mom's chest
if just tachy and with the servo probe and radiant warmer off toward them to
eliminate the draft from the ac and babies often resolved their respiration
rates more quickly....and then they would start to cue for Mom....A really tachy
baby isn't usually interested in latching while they are trying to catch their
breath after delivery...it's often the family members who are saying "So when
can he breastfeed?"  After explaining the baby is settling down and we need to
let the baby give us the information if he is ready to feed, that's when he
will show us he is ready."  Most babies resolved within an hour and if the baby
became more symptomatic then we would ask the neo to come to the bedside and
evaluate...yes, some made it to NICU and others went on to be cuddled and held
through out the transition.  Now I work in a place where all babies are in a
transition nursery....my goal in the next few months is just to get the kids
to breastfeed in the LD room....unfortunately the Cesarean section mom's don't
get to see the baby except briefly in the OR...in PACU they are usually
vomiting or cold...I now have honor of working in the CS capital of SFL
.....Yesterday 7 scheduled sections...3 were able to breastfeed after baby came out.
Those of us who are the breastfeeding "freaks" (said proudly) are looking at it
like it not as a job, but as an adventure.   Our breastfeeding taskforce seeks
to keep the NSVD's ( is it a NSVD if she had an epidural???) and baby together
and breastfeed...we are keep stats now...needless this ticks off some LD RN's
who feel its their job to get the baby to the nursery and mom to the
Mother/Baby units....Our plan is to get them to remember that "Golden Hour" and every
Mom and Baby deserve it at very least no matter how busy they are....its Mom's
reward for a job well done.  I just wonder where they (LD RNs ) lost the
concept of Moms and Babies Together.  I think I might see a few less SAT monitors
going in nursery, if Mom could just let baby settle down on her and take their
time becoming the couplet they are...but Nursing Management will have to
change their current thinking.
