Susan, I have worked with two mothers with silicone implants.  One had the
implants after she had two children and didn't plan to have another.  She saw
me in her pregnancy and we talked about her plans to breastfeed.  She
successfully breastfed and didn't supplement.  Her implants had been inserted
from under the breast and placed behind her ducts, so there seemed to be no
problem with breastfeeding, supply etc.  The other mom I have worked with had
implants as a young adult and never thought about breastfeeding.  She also has
done wonderfully with no supplementation.  The last time I talked with her she
was pumping and finding the pump uncomfortable, but I think the discomfort was
from the type of pump, not her implants.

As for the concern of silicone hurting the baby, from what I could find it
seems that it would be rare for the silicone to be in the milk ducts if the
implant leaked.  If the silicone did somehow get in the milk ducts, it would
enter the baby's body through the digestive system which wouldn't hurt the
baby, as it would simply be excreted without harming the baby (told to me by
an MD who pointed out that we give babies silicone-based meds for gas).  I did
find a reference to testing the milk for silicone..don't know how much that
would involve or if it is worth the trouble.
