while some solutions may work for this lady and her baby. it could be a case
of the baby is doing some thing add with the tongue that is not precisely
correctable in the way we may like to think.

Careful observational skill, support and listening to the mother is
paramount. with all the usuall attachment tricks as suggested by winnie and
kathleen could very well improve things somewhat. But if they don't it could
be something the baby is doing that no one has full control over.

i have followed at least three babies who do such things and no amount of
teaching and training and technique fixed the problem. However with support
two mum's were able to continue breastfeeding beyond 6 months. By expressing
and giving the baby  EBM in a bottle using a rubber teat that is about 1/4
inch longer than most other teats (so it reaches to the back of the junction
of the soft and hard palate and at least continues to stimulate that spot
and the rubber fills the mouth see article by robyn Noble in Breastfeeding
reveiw) the baby gets fed and the pressure is off mum. she needs to continue
to try to get baby latched well at every feed but if it doesn't work then
just feed the EBM by bottle using particular teat and techniques that would
help tongue coordination  and gape.

some babies do learn a coordinated suck pattern from this method. others
never do. One baby I saw is now 14 months old and still taking 1-2 breast
feeds per day. His mum tells me he still ridges his tongue in the middle and
causes her nipple to be squashed during the feed and causes some pain but
not as bad as in the early days. she found that the feed in the early
morning was the least painful as she would feed lying on her side in bed and
her breast very full caused different dynamics for the baby in the way he
manuvouered his tongue. this baby  always clicked with the feed as well.

a second baby is now 8 months old and mum called me in desperation at 6
months after bearing pain for that long. she wanted to feed longer for the
full 12 months at least. so after careful assessment i told her i could not
promise much change but suggested lying down in the morning and see what
happens. she rang me the next day to say she had the first painless
breastfeed. This was sufficient for her to keep going a little longer and
she is happy doing a few breastfeeds and some bottle with EBM.

hope that is of some use it may not solve the problem but could help
continue to see what works best for that mum and bub
