Hi Ann,
I agree that this infant probably has developmental/neurological
issues.  I would not discourage the mom from feeding on both breasts,
since at a year her supply is already calibrated (possibly to a low
level), and as long as she lets the baby nurse on the first breast as
long as she wants, she will get more milk by feeding on both sides. The
best way to increase the fat content of the milk is to feed more often.

Trouble handling solids, especially purees (the foods that are typically
spoon fed) points to an oral motor problem.  Ask if the baby also has
trouble with mixed phase foods (soups or stews).  If so, it could be as
simple as a tongue tie, or as complicated as a neurologically based
problem with tongue movement.
