At one of the hospitals where I teach BF classes our LCs are having a discussion about pacifiers.  Typically, I say in BF classes to wait 4-6 weeks before introducing bottles or pacifiers, since I have felt this was the consensus of most LCs.  However, it seems that most of the hospital LCs are saying it's ok to introduce 'other nipples' at 2-3 weeks.  
 My reasoning has been that milk production is not stable til around 4 weeks, many babies still do not have a mature rhythm, and sophisticated suck/swallow coordination and strength may not be developed until around that time.   AND, I have no moms calling me to say that their babies are refusing a bottle at 4 weeks of age, in the pedi practice where I work as an LC.  Later at 8 weeks and beyond the calls do come in that baby is refusing bottles.  Moms who find baby refuses a pacifier at 8 weeks do not really consider this a problem.
   I will be reading archives to see how this was treated in the past.   Anybody who has any research to support the delay to 4-6 weeks would be given my next born child... (I'm 57 yo)...    TIA     michelle scott, MA, RD, IBCLC
