I agree with Mimi.  The policy described seems unusually harsh.  We try to 
use breastfeeding as a "carrot" to help moms stay off drugs instead of 
withholding breastfeeding as a punishment for bad behavior.  We have no set policy - 
each pediatrician can decide on a case by case basis.  Unless you plan on 
removing the infant from the home (and in California it takes a lot more than just 
substance abuse) you should not be restricting breastfeeding in the hospital.  
In the NICU also, we also go on a case by case basis.
